Well, I was reviewing medical records today and I came across two huge blunders that absolutely had me and others laughing. So here they are:

The patient sustained a right shoulder injury in a carcinoma accident.

Status post right shoulder rotator cough injury.

Well, what do you think of that? It seems that some voice recognition software was used for these. If people get to the point that they don’t care what appears in a medical record/report, then we will be coming across these on a daily basis.

I sure hope that is not the case. But you can see from the above examples why we need good medical transcriptionists who will use their brains when doing this type of work.
